a] Monthly Medicines
-- Med can be purchased in one go to last till 28 Feb 2021 or month wise.
Med purchased in the month of Feb should only be for 30 days.
The med purchased should be based on prescription given by MO/ Specialist of Polyclinic/ service hosp/ Empanelled Hosp/ sehatopd.
b) Submit reimbursement
claim by Regd post, Courier or drop at Drop box. Postal address: ECHS

c)Temp
Slips are valid upto 30 Jun 21, not
required to go to Polyclinic for renewal.
d) Malignancies requiring
Chemotherapy or Radiotherapy, Chronic Kidney Disease requiring Dialysis,
Cardiac ailments requiring frequent follow up at Empaneled Hospitals or
Treatments requiring frequent intraocular injections can
be availed as EMERGENCY from Empaneled / Non Empaneled facilities, without Referral from the PolyClinic.
Visit the Polyclinic, by taking Appointment on Mob No 95525 84587 from 1300 - 1530 hrs at least one day earlier.
WE HAVE NOW MIGRATED TO NEW 64 KB SOFTWARE:
1. All ESMS are requested
to get their cards & temporary slips checked for acceptance by the new
software system at the PolyClinic.
2. In case their card/slip
is not accepted by the new system, they need to find out the cause & Check status at https://echs.sourceinfosys.com/ or download ECHS Beneficiary App from Google Play store. ECHS Website is https://echs.gov.in
3. ESMS who have received
earlier intimation regarding observation on the status of their cards, the same
needs to be acted upon as soon as possible.
Above 75 years of age and their dependants can directly visit Specialist at MH
on OPD days, without referral from ECHS PC.
